Health Navigator (Social Worker) Full Time Title: Health Navigator (Social Worker) Status: Full Time Shift: Daylight, Monday - Friday, on-call rotation Location: Greensburg What does Redstone offer me? As a Health Navigator (Social Worker) , supporting the Redstone mission will include the following responsibilities: Performs services of social service for assigned clients of the Skilled Nursing Facility. Social Service functions include but are not limited to: Coordination of hospice referrals; coordination of psychological and psychiatric consultations; SOS (complaint) process; weekly calls to families of assigned skilled clients to provide an update; attending care conferences and documenting progress notes as necessary. Completes assigned section of the MDS assessment. Reviews charting and enters data regarding client's psychosocial information, behaviors, and moods into the MDS system. Coordinates admission process for assigned clients of the Health Care Center including meeting with resident/responsible party to complete resident forms. Coordinates transfers between levels of care throughout the organization with assigned clients. For assigned clients, communicates and coordinates discharges from the Healthcare Center and/or transfers to other levels of service within the ASN. This responsibility includes working with the responsible party of each client based on interdisciplinary team discharge recommendations and completing the necessary paperwork for Home Health/Home Care/Hospice referrals. Coordinates with Business Office Manager the Medical Assistance application process from start to approval. Obtains the necessary documentation from the physician, client/responsible party, and County Assistance Office (CAO). Responsible for the achievement of individual sales/volume goals as developed and prioritized. Participates as a member of the Interdisciplinary Team (IDT) to coordinate services for assigned clients throughout the ASN. Provides weekly updates on assigned clients who are hospitalized to the IDT team. What do I need for this role with Redstone? Master's degree (MSW) from a school of social work accredited by the Council on Social Work Education Bachelor's in social work Five (or more) years of related experience in a health care setting (preferred) One year of experience in home health (preferred) License in Social Work Must possess valid driver's license What makes Redstone unique?
