Serving approximately 19,633 annually and providing 1,861 units of supportive housing last year, LOFT is one of Ontario’s largest mental health service providers of its kind. LOFT is also recognized as a Nonprofit Employer of Choice™ Award (NEOC) recipient. To help fulfill its mission, LOFT is hiring a Intensive Case Manager. Job Summary As an Intensive Case Manager you will be part of an interdisciplinary team that co-ordinates a support network and links individuals living with multiple complex mental illnesses, substance use challenges and physical health issues, and the social drivers of health. The position attempts to empower the individual thereby increasing their independence, dignity and self-respect in a safe and secure environment. Direct service and support will be provided to clients in areas such as daily living, practical assistance, crisis intervention, linkages to primary care and a range of other services to support the individual’s needs. Note : Staff will be required to work in residents’ rooms/apartment units and may therefore be exposed to secondhand smoke. There may be occasional evening and weekend hours. Social Medicine Initiative is an interdisciplinary team of providers from the University Health Network and LOFT Community Services that provides wrap around services to individuals with concurrent conditions that span physical and mental health challenges and social and economic vulnerability who are experiencing homelessness or at risk of losing their housing.
