Medical Assistant Position Summary: Eventus WholeHealth delivers an integrated model that provides holistic care for medically vulnerable adults achieving extraordinary outcomes for the people we serve. This care is provided to adults who reside in skilled nursing, assisted living facilities and private residences through a network of healthcare providers including Physicians, Nurse Practitioners, Physician Assistants, Psychologists, Licensed Clinical Social Worker, Clinical and Administrative Support Staff. TheMedical Assistant is responsible for enhancing the performance of theclinical care teamthroughsupporting and facilitating clinical services with their assigned providers. The Medical Assistant is required to work in the field with the assigned provider as directed by Lead Medical Assistant or MA Manager/Supervisor.You may be asked to travel to facilities up to 60 miles from your home. Hours:Mon-Fri - 8 hour dayshift (flexible) * Mileage reimbursement plan is offered. Provider Support Ensure thattheclinical provider in the assigned buildingisable tofocus on taking care of their patients for the day Ensurethat each patient is seen asmedically necessaryperclinical protocol. Serve as a "buffer" between provider and families/staff to facilitate proper patient management. Document appropriate encounter and timeforCCMand BHI services If scheduling is assigned: Scheduling rosters should be updateddaily to ensure accuracy and schedules sent timely. Assist with any day-of changes to the scheduled visits due to acute needs, hospitalizations, admissions, or discharges Prepareclinical documentation for assigned providerseach day Completebasic opening of encounternoteas per discipline protocol Perform medication reconciliation on all scheduled patientseachencounter Enterappropriate facility chartdataon all scheduledpatientseach dayinto patient encounternotesand assign to provider Enter lab and other diagnostic study results into patient encounter and assign to provider as per guidelines Assist withcollectionand sorting ofall clinical paperwork needed for clinician review and ensure completion each day Host telehealthsessionsfor providers as assigned by manager/supervisor, both routine and acute visits Facility Support When scheduling is an expectation for your role: Share patient schedules with key facility staff as needed. Assist with anytriage needs as directed by assigned provider (whether they are in or out of the building that day) Assist with triage documentation as needed. Ensure that all needed paperwork is routed for the facility to Eventus providers, office, or families. Assist with consents for any services needed. Attendstand up meetings to reportany needed information from the facility toEventusprovidersand vice versa if allowed by facility. Assist with discharge planning of clinical servicesas needed. Ensuring check in and out each day with facility staff regarding patients seen, status, etc. Patient Support Maintain patient confidentiality at all times. Document appropriate encounter and timeforCCMand BHI services Check all forms of communicationseveral times daily to ensure patient needs are met. Inform patientand family ofrecent visits, new orders and defer to provider when needed Communicate transitions of care to CCM team to enhancepatient care and education. Additional Duties and Responsibilities Medical Assistantsare expected to be knowledgeable and adhere toEventusWholeHealthstandards, policies and procedures and outlined in employee handbookincluding HIPAArules andregulations. Medical Assistantsshould adhere to theEventusWholeHealthdress code policyi.e.clean, wrinkle-free, matching scrubs with appropriate, closed-toed footwear and name badge. When duties are completedahead of scheduledworkday, report to your Lead Medical Assistant for any additionalassignments. Medical Assistants should seek further training if uncertain how to proceed with any assignment.
